Patient may have a wide range of symptoms from bloating , abdominal pain and fever to jaundice in extreme situations. In symptomatic patients surgery is the ideal treatment option. In patients with asymptomatic stones detected incidentally observation is a viable option until they become symptomatic.Currently almost all Gall bladder surgeries are performed by key hole route. Some hereditary and autoimmune causes have also been related which are on the rise especially in younger age group. Early diagnosis and treatment is required in order to avoid long term complications which include diabetes, indigestion, chronic diarrhea and chronic pancreatitis. Management is to prevent exposure to offending things. that aggrevate the attacks. Advanced management includes endoscopic procedures. Surgery is reserved for cysts that occur due to recurrent attacks.
Liver is an important organ which plays a vital role in digestion of food, breakdown of medications and other toxins that are accidentally ingested. With a variety of functions, the risk of diseases the liver faces is also high. With advanced scans and blood tests even small liver swellings are picked up. Ironically all of these appear similar and needs an expert to plan further management as the thought of possibility of a cancer cannot be excluded very easily. Another set of disorders that affect the liver are Hepatitis and Alcohol related disorder called Cirrhosis both of which cause generalised changes and complications all over the body. It takes multidisciplinary approach to sort this out. Most liver diseases are diagnosed late as liver has the ability to compensate until majority of the liver functions are altered. Hence early intervention is adviced.
Diseases of the Small intestine are rare and difficult to diagnose. They present with bizarre and confusing symptoms like abdominal pain , weight loss, anaemia, loose stools and weight loss. In the absence of a swelling and long intestinal length it is very difficult to isolate the location of the disease. Disorders range from infections to cancers.. Most disorders present with intestinal blockage or bleeding. Early investigations are required to manage symptoms and prevent progression of disease.
The colon is an organ of great length and diameter. It is a reservoir for storing stools , concentrating them and absorbs water making the stools solid. Having wide range of functions, the colon is exposed to a lot of stress from the toxins in food and infections. Due to their large diameter, distensible nature and deep location early disease is not symptomatic. It takes a certain level of progression for the colonic diverticulum to turn symptomatic and by then its often advanced. Hence its advised to seek medical help at the earliest. Advanced and complicated diverticulum will need surgical management.
Irritable bowel syndrome or irritable bowel disease is a disorder which causes bowel irritation resulting in inconsistent stool habits. Some people have urgency to pass stool after meals or a stressful encounter while some others have episodes of loose stools. It requires control of stress or control of irritable food substances which cause these symptoms. Most people feel depressed with the long term existence of symptoms. Inflammatory bowel disease encompass a spectrum of disorders with patchy inflammation of one or several parts of the intestine. They may present with intestinal or non intestinal symptoms often leading to severe pain, alteration of bowel habits, bleeding in stools and sometime ulceration, rupture or intestinal obstruction. Initial management will be supportive care but patient needs evaluation to differentiate an inflammatory bowel disorder from other disorders. Presence of severe pain or bleeding or perforation may present as an emergency while some other symptoms may mimic an infection or a cancer. Evaluation, identification and long term supportive management with diet and medications are in order whereas surgery is reserved for patients with severe bleeding or obstruction.
colonic and rectal cancers management have had a tremendous change in the last 2 decades. On the surgical front key hole and robotic surgeries have taken the leap and addition of chemotherapy and radiation to rectal cancers have improved surgical outcomes and disease survival. Another area of advancement is offering chemotherapy to colorectal cancers that have an advanced presentation with liver spread.
